The service provides urgent and same day response (sometimes as quickly as within 2 hours from receiving referral) to support people to remain at home and reduce admission and conveyance to hospital. This service can provide equipment urgently and care if necessary for a short period of time to help patients remain in their own home.

The primary aim of the service is to support patients with physical health needs such as falls, chest/urine infections, infective or non-infective exacerbation of COPD, long term conditions and palliative conditions.

The service operates 365 days a year from 8am to 8pm and works with Lancashire Teaching Hospitals and East Lancashire Hospital Trust, to facilitate discharges back into the community.

Patients must be registered with a Blackburn with Darwen GP and be over the age of 18 years. Referrals are accepted via telephone call into a single point of access for Pennine.

The service consists of the following staff:

  • Nurses
  • Occupational therapists
  • Physiotherapists
  • Health care assistant
