The Rapid Assessment and Community Rehabilitation Team work within the Central Lancashire community to support clients that have been had a deterioration in their abilities to manage at home as a result of a change in their health and wellbeing.
Please note: this service is only available in Preston, Chorley and South Ribble.
The service provides a response within two hours to reduce avoidable hospital admissions and can provide rehabilitation programmes to enable clients to remain independent in their own home. The rapid assessment team can provide equipment urgently and care if necessary for a short period of time to help clients remain in their own home.
The aim of the service is to support clients who have a physical health problem such as falls, chest/urine infections and palliative conditions.The service operates from 8am to 8pm Monday to Friday and 8am to 5pm weekends and bank holidays.
Clients must be registered with a Central Lancashire GP and be over the age of 18 years. All clients will receive a telephone call triage before an assessment at home.
The team has various bases across Central Lancashire. The team will see clients in their own home, including sheltered accommodation and temporary accommodation, and in the hospital site, at Chorley and Preston in A&E and RAU/MAU.

Referrals must come via a health or social care professional. We do not accept self-referrals at this time.
