Wordsworth Terrace Supported Accommodation
Wordsworth Terrace is a 24 hour staffed supported accommodation for adults with mental health problems requiring rehabilitation of their daily living skills.
The service houses 12 people in three separate units, two male and one female.
People are referred to us because they have got to a point in their life where they need a period of intensive support to develop or improve their skills in managing day to day life. A rehab care plan will be devised with the individual to identify goals and how to work towards them with staff support. Such things may be managing personal and domestic care tasks, building a meaningful daily routine, managing money and correspondence, or accessing the community and its many opportunities for social, leisure, work or educational activities.
The staffing team consists of an occupational therapist, nurse, and support workers who all work closely with family members and the wider teams and services that may be involved with the individual.
Optimum length of stays can be variable depending on the levels of enthusiasm and motivation as well as the pace of the individual. This may often be between 6 and 18 months.
How to access the service
Referrals come from wards and mental health care teams where individuals have been identified as being unable to return home at this time but there is hope and potential for regaining skills or learning new ones with the support of the experienced team members. Individuals may move onto independent living with or without further support, or to step down services.
Where is the service provided?
The service is situated within Chorley, about a mile down the road from Chorley Hospital. We accept people from all over Lancashire except for the unitary authorities Blackpool and Blackburn with Darwen.
