Forensic Community Mental Health Team
The Forensic Community Mental Health Team (FCMHT) are based at Fellside Ward, which is a mens’ low secure pre-discharge ward. It is located at Guild Lodge which is a secure mental health service. Active rehabilitation and discharge planning are the focus, with many of the service users in the final stages of inpatient care before they return to life in the community.
FCMHT is a multi-disciplinary team of Nurses, Social Workers and Doctors. The team functions on a 7 days a week, 24 hours a day basis, with an on-call rota between 5pm and 9am, as well as throughout the weekend and public holidays. The team manages a caseload of approximately 60 patients who are on extended leave from, or discharged from Guild Lodge.
The team also links in with each of the inpatient wards at Guild Lodge, providing designated health and social care staff to all in patient areas for advice, support and pre-discharge planning. As the majority of the team are social workers the team prepare and contribute to a significant number of statutory reports.
How to access the service
Referrals come to the Forensic Community Mental Health Team from within Guild Lodge when it is envisaged that a service user on a particular ward area is approaching potential discharge in a timescale of approximately 6 months. Once accepted, a CPN, Social Worker and Responsible Clinician are identified for that individual, get to know the service user, plan and oversee the gradual transition from in-patient to community living.
Referrals to Fellside come from other wards of higher security or different remit within Guild Lodge when it is agreed that a move to conditions of a less restrictive and more rehabilitation-focussed nature are indicated.
