Self-referral is for everyone aged 16 or over and registered with a GP covered by Mindsmatter.

If you are a health professional and you would like to make a referral, please contact the local team. Please do not complete a self-referral form on behalf of someone else.

Mindsmatter is not an emergency service, if you are in mental health distress and need urgent help you can call our crisis line on 0800 953 0110 or Samaritans on 116 123 24 hours a day, 365 days a year.

When I self-refer what happens next?

Once your referral has been accepted by your local team they will then be in touch to book your assessment. We usually do the assessment over the phone. If you think that you will struggle speaking on the phone, please let us know when we contact you to discuss an alternative appointment.
