The community living skills mental health team supports individuals with severe, enduring and complex mental health problems who require a period of intensive rehabilitation in their own home to improve or maintain their independence or to prevent acute inpatient admission.

The team consists of occupational therapists and support workers who will, following a comprehensive assessment of function, provide intensive rehabilitation for people who have difficulty initiating, sustaining or managing some, or all, aspects of their day to day living.  We aim to enable them to improve their activities of daily living such as personal care, domestic care, correspondence and financial management, using transport, accessing community activities, and exploring work related or educational opportunities.

How to access the service 

The service can be accessed via a referral from the service user's care coordinator, usually within their community mental health team. This is not a self-referral service. All referrals will require a current risk assessment and health and social needs assessment.
