Neurological Rehabilitation Team

The team provides specialist multi-disciplinary input for people with neurological conditions in Central Lancashire and Blackburn with Darwen. Rehabilitation programmes are designed specifically for the individual and are centred on meeting agreed client goals. Therapy is carried out in patients' homes or other suitable community settings.

The team consists of:

• Speech and language therapists
• Physiotherapists
• Occupational therapists
• Rehabilitation assistants
• Clinical psychologist
• Administration staff

How to access the service

Patients are referred to the service by their GP or other treating health or social care professionals (in hospital or community settings). All referrals are considered on an individual basis, taking in to account presenting needs and potential for rehabilitation.

Who do we provide a service to?

• Patients who have clear potential for further significant functional gain and are medically stable
• People registered with a GP within Greater Preston or Chorley and South Ribble areas
• Patients must be aged over 16 years old
