Albion Mill is a 35 bedded Intermediate Care Service which aims to provide high quality, person centred, community based, short term intermediate care to residents from their own home as step up and to those being discharged from hospital as step down.

The service shall be considered for all Blackburn with Darwen residents who are being treated for an exacerbation of a condition, trauma or illness, which has resulted in a need for rehabilitation or recovery and where this cannot be delivered in a person’s own home. The service shall empower people to recover from episodes of ill health and to return home through the provision of individualised support.

It will be led by a multidisciplinary team designed to optimise an individual’s level of independence based on identified needs and in accordance with the agreed care plans. The team will provide 365 day cover 24 hours per day. Of the 35 beds, 16 beds will be for people with identified nursing needs.

The purpose of Albion Mill is to provide high quality therapy and nursing care, to promote fast recovery from illness, to prevent unnecessary hospital admissions and premature admission to short and long term care by managing the nursing and therapy needs of patients and working in collaboration with the multi-disciplinary team.
