Single Point of Access for Community Services

The Single Point of Access (SPA) is utilised as a referral point for the following services in Blackburn with Darwen:

• District nurses
• Occupational therapy
• Pulmonary rehabilitation
• Integrated neighbourhood team
• Intensive home support services (which includes the following services)
• Rapid assessment team
• IV therapy
• Complex case managers
• COPD service

All referrals will be triaged and acted on appropriately, dependent upon need.
