Hello,
Thank you for taking the time to speak with us.
You are on one of our service waiting lists, awaiting an appointment. We apologise for the amount of time you have waited and would like to inform you that we are working hard to see and treat patients in order of clinical priority as quickly as we can.
The purpose of contacting you is to ask you to confirm if you wish to remain on the waiting list. If you wish to remain, you don’t need to call us as you will be contacted by the service when an appointment becomes available.
To start this virtual conversation, we will need to verify your details by clicking continue.

We are monitoring our waiting lists and as part of this process we are contacting patients to check they still require an appointment or wish to be removed from a waiting list.
- Your response will help us keep our waiting lists accurate and ensure we are able to see and treat patients who need to be seen sooner.
- A wasted appointment, which a patient does not attend, could be used for someone who is in more clinical need.
- You don't need to do anything if you tell us you want to remain on our waiting list. You will be contacted when an appointment becomes available.
- Some patients tell us that they have already received treatment elsewhere or no longer need the appointment, in which case we will remove you from the waiting list, and you will receive a letter confirming this. However, if it is not in your best clinical interests to be discharged, you may need to remain on the waiting list, and we will inform you of this decision.
Unfortunately, we are unable to give you a date for an appointment, but please be assured, we will contact you when one becomes available.
As soon as an appointment becomes available, we will either call or write to you, inviting you to attend an appointment. If the date and time are not convenient for you, you will be given the opportunity to change them.
Please make every effort to attend your appointment or contact the service if you are unable to attend.
Failure to do so may result in you being discharged from LSCft and back to your referrer.
If you select the wrong option within the virtual conversation or have changed your mind, please contact the service on the number provided within the assistant in the next 5 working days to update your response.
